HOW TO USE THE MEAL PLAN
December, 2010
1)    Choose the days you wish your child to receive a school lunch by circling the choice of meal. A form must be filled out for each child.
2)    Meals are $3.50 each, but it is up to your discretion how much you feel you can afford to pay for the lunches. Payments will be kept confidential. We prefer any payments to be made by Wednesday, November18, 2010, but payments can be made at the office any time during the month. Whatever you can pay is appreciated as the program depends on donations and payments.
3)Â Â Â Â Please make comments if you wish, about the program in the space provided below.
4)Â Â Â Â Return the completed menu and any payments to the school.
5)Â Â Â Â Return this order form to the school office by, Wednesday, November18, 2010. Envelopes returned after the 18th cannot be accepted for the December meals program.
Again we want to stress that any donation or payment that you can make is appreciated and needed. We understand if you cannot but we need people who can afford to pay, to do so or at least a portion of. The program depends on this.
